An adult can legally change their name by entering into a change of name deed which is a document that needs to be signed and independently witnessed.
A parent with parental responsibility for a child can enter into a change of name deed for the child provided they have the consent of any other person who shares parental responsibility for the child or a court order allowing the change of name.
